This is the consent form required for our Background Check program. Please fill out & email to MountainTopChrysalis@gmail.com or hand deliver to any Mountain Top Chrysalis event. In order to standardize out background checks, we are asking that all checks be run through our program. The cost is only $15.

It is a requirement of the Upper Room that all Spiritual Director fill this form out and submit it to the Community Spiritual Director (Bo Phillips). This form is kept on file, so it is only necessary to submit it once.

Scholarships are available on an as need basis. If you need one for registration of a Caterpillar, Team Fees, or Big House fees, please fill out the form and submit to the correct person.
